Application Process

All applicants must participate in computer matching. Applicants should request instructions and mark/sense card to prioritize their program  preferences from:

This request should be made to allow turn-around time to submit the mark/sense card by the mid-February deadline. There is no charge for this
material. However, there is a $50.00 charge for computer matching that is due with the applicant’s prioritized ranking when submitted to D & D
Digital Systems.

For the University of Maine application to be considered in the first round match, applications must be received by January 31. Applicants must
be accepted to graduate school to be considered for an internship position. Applicants will be notified of the decision of the graduate school prior to internship notification in April.

Once applicants receive notification from D & D Digital System regarding their acceptance or non-acceptance into the Program, within 24 hours they must email awhite@maine.edu, Program Director, their decision to accept or decline the appointment.

Please read and follow all directions carefully, as incomplete applications are not considered.

Following are the materials required for application:

1. Completed Graduate School Application. The degree sought is either a Master of Science or a Doctor of Philosophy. This form can be filled out
on-line.

2. Recommendation letters/forms. You need 3 letters of reference. Provide each person making out the reference with 1) the letter of recommendation to The Graduate School form (found on-line) and 2) the current edition of
The American Dietetic Association’s Waiver and Recommendation Form. The evaluator should place both reference forms in a sealed and signed envelope.

4. Official Transcript(s) from all colleges and universities attended.

5. One original copy of "Declaration of Intent to Complete Degree" or "Verification Statement of Completion of Program" (Whichever is appropriate for your situation). The verification form must be forwarded to the Adrienne White, internship director, when you receive a copy from your university's DPD director.

6. Graduate School Application fee. A $60 non-refundable application fee is due with the online graduate school application ($75 for paper application). Your check or money order should be made payable to the University of Maine and sent to the graduate school. http://gradcatalog.umaine.edu/apply/index.php

All application materials for graduate school, including the internship application materials, need to be received at The Graduate School by
